Media Release
I, the undersigned, hereby grant permission to Celebrate the Arts Performance
Academy, Inc. (CAP Academy) to photograph, video record, and/or use images of
myself and/or my child for educational, promotional, and marketing purposes.
This includes, but is not limited to:
Social media platforms
Website (capacademycle.org)
Flyers, brochures, and marketing materials
Grant and funding report
I understand that:
These images may be used without further notice or compensation
CAP Academy will use all media respectfully and professionally
Names may or may not be included
I release CAP Academy, its staff, partners, and affiliates from any claims related to the
use of these images.

Program Payment Agreement
This agreement outlines the payment structure for participation in CAP Academy
programs.
Program Fees
Fees vary by program and may include:
Tuition (weekly, monthly, or full session)
Registration fee (non-refundable)
Materials or supply fees (if applicable)
Payment Schedule Options
Participants may select one of the following:

Liability Waiver Release
I acknowledge that participation in programs offered by Celebrate the Arts Performance
Academy, Inc.,
including but not limited to:
Performing arts (dance, music, theater)
Urban agriculture and gardening activities
Workshops, field trips, and community events
May involve physical activity and inherent risks.
I voluntarily agree to the following:
Assumption of Risk
I understand and accept all risks associated with participation.
Medical Consent
In the event of an emergency, I authorize CAP Academy staff to seek medical treatment
if I cannot be reached.
Release of Liability
I hereby release and hold harmless Celebrate the Arts Performance Academy, Inc., its
staff, volunteers, partners (including Urban Queen Bee LLC and partner sites such as
churches or schools), from any and all claims, injuries, damages, or losses.
Behavior Expectations
Participants are expected to follow program guidelines. CAP Academy reserves the
right to dismiss participants for unsafe or inappropriate behavior.
Outdoor & Agricultural Activities
I understand that gardening and outdoor programs may include exposure to:
Weather conditions
Insects (including bees)
Tools and soil-related activities
I accept these conditions as part of participation.
